Housing and Street Outreach

A Continuum of Care

Project H.O.M.E. has developed a proven and effective program to assist persons in overcoming chronic homelessness. This "continuum of care" is carefully designed to deal with the complex issues of persons with special needs such as mental illness and addiction. The continuum consists of street outreach, a range of supportive housing from entry-level to permanent housing, and comprehensive services, including health care, education and employment.

If you see a homeless person living on the streets, please call our Homeless Outreach Hotline at 215-232-1984.

If you are client in need of services, please call 1-877-222-1984.
